INFORMED CONSENT FOR PERSONAL TRAINING TERMS & CONDITIONS
I understand that I have given consent and will be taking part in Personal Training.
I understand that during personal training there will be intensity changes occurring. I understand that I may stop at any time due to fatigue, changes in my heart rate and or blood pressure. I understand that I may stop whenever I wish because of feelings of fatigue and or any other discomforts.
ATTENDANT RISK AND DISCOMFORTS
There exists the possibility of certain changes occurring during personal training.
They include abnormal blood pressure, fainting, irregular, fast or slow heart rhythm.
TCM & Korean Hand Acupuncture & Acupressure
I understand if I decide to take part in any holistic therapy/treatment that it is an uninsured activity and removal all liability from West London Personal Training (WLPTLTD) and its employees.
I understand TCM & Korean Hand Acupuncture involves the use of needles and consent fully to allow the practitioner to insert needles into my body using TCM & Korean Hand Acupuncture diagnostic tools.
The use of TCM, Korean Hand Acupuncture & Acupressure may leave some swelling, blood spotting and or localised bruising, I understand this and fully consent to these side effects.
I understand that every effort will be made to minimise these risks by evaluation of preliminary information relating to my health and fitness and by observation.
RESPONSIBILITIES OF THE PARTICIPANT
I understand that information I possess about my health status or previous experiences of unusual feelings with physical effort may affect the safety and value of my fitness test. I acknowledge that my prompt reporting of unusual feelings during the exercise test itself is extremely important and that I am responsible for fully disclosing such information whether or not requested by the testing staff.
I understand I will be making changes to my diet/nutrition and have been given professional and medical approval from a qualified medical Doctor prior to starting and engaging in any offline or online nutritional service provided by West London Personal Training.
I understand I will be participating in strenuous physical activity and have been given professional and medical approval from a qualified medical Doctor prior to starting and engaging in any offline or online exercise/personal training service provided by West London Personal Training.
I fully understand my responsibilities as the participant and am using services provided by West London Personal Training in accordance to the services terms and conditions and at my own risk.
BENEFITS TO BE EXPECTED
I understand that the results obtained from exercise training may assist in evaluating what type of physical activities I might do with low risk and be used as a guidance for my fitness level progression.
I acknowledge that questions about the procedures used in the fitness test or the results of my test are encouraged. If I have any concerns or questions, I understand that I should ask for further explanations.
TRAINING CANCELLATION NOTICE
I understand that West London Personal Training requires 5 hours notice on the day of training to cancel a pre booked session, failure to provide notice or not show up for my session will result in a fully charged personal training session.
I understand that in the event I have pre paid for sessions in advance, I will forfeit/lose a session should I not turn up to my session or if I have given less than 5 hours notice.
CHANGE OF TERMS & CONDITIONS
I understand that West London Personal Training reserve the right to change the terms of condition(s) at any point in time.
RELEASE OF LIABILITY
I have voluntarily enrolled on to a program of strenuous physical activity including, but not limited to walking, running, boxing, weight lifting, cycling and the use of various conditioning equipment and facilities designed, offered, recommended and or supervised by West London Personal Training. I hereby affirm that I am in good physical condition and do not suffer from any disability that would prevent or limit my participation in this program.
In consideration of my participation in the program, I for myself, my employees, heir, trainers, assigns, agents, officers, directors, shareholders, from any and all claims, demands or causes of action arising from my participation in the program or from any use of the conditioning equipment/machines/equipment(s) and facilities.
I fully understand that I may suffer injury as a result of my participation in the program and I hereby release West London Personal Training Inc, from any and all liabilities now or in the future, including but not limited to medical expenses, lost wages, pain and suffering, that may occur by reasons of heart attacks, muscle strains, pulls or injury, however caused, whether occurring during or after my participation in the program or use of the conditioning and exercise equipment and facilities, regardless of fault.
I attest, contract, acknowledge and agree that I am legally bound by its content.

You will NOT hold any third party agent or organisation whom you purchase the My PT PAL service from or through, will be liable for:
any physical injuries, mental conditions, business or personal income losses, such as loss of profits, income, revenue, anticipated savings, business, contracts, goodwill or commercial opportunities; loss or corruption of any data, database or software; any special, indirect or consequential loss or damage.
WLPT LTD will not issue and are NOT OBLIGED to offer refunds for the differences of purchase costs to any customers under any circumstances. You agree and consent to these terms and conditions when you purchase the My PT PAL service.
Limitation of Liability
Nothing in these terms and conditions will: (a) limit or exclude our or your liability for death or personal injury resulting from our or your negligence, as applicable; (b) limit or exclude our or your liability for fraud or fraudulent misrepresentation; or (c) limit or exclude any of our or your liabilities in any way that is not permitted under applicable law.
